Examples of medical documents for an insurance claim in Bulgaria

These samples show the medical documents normally issued by clinics, doctors and pharmacies in Bulgaria. They can support an insurance claim after you have paid for medical care yourself.

Before you leave the clinic, doctor or pharmacy: ask for the medical document and the financial document. Formats can differ, but the information matters. Documents must be signed and stamped.

1. Invoice

An invoice (фактура) records the services provided and their price. It should be issued together with a fiscal receipt (фискален бон) for the same amount. Sometimes the receipt is attached to the medical certificate or outpatient sheet rather than to the invoice.

What the invoice should contain

document number and date;
name, address and identification details of the clinic or doctor;
services provided, their price and the total amount;
name, address and personal details of the patient;
signature and stamp of the doctor or authorised representative.

If a doctor cannot issue an invoice, ask for a cash-in receipt. A pharmacy must issue an invoice when asked.

2. Outpatient sheet

An outpatient sheet records the consultation, diagnosis, treatment and prescribed medicines. It contains patient data, the date, medical institution and doctor details, signature and stamp.

3. Medical certificate

A medical certificate can serve the same purpose as an outpatient sheet. It should show the patient, diagnosis, treatment, prescriptions, date, medical institution and doctor details, signature and stamp.

4. Epicrisis / clinical report

An epicrisis is a clinical report describing the patient’s condition, diagnosis, medical history and treatment results. It is usually not required for a straightforward case. It can be required when the insurer needs detailed medical explanations, for example after hospital treatment or where a chronic condition may be involved.

5. Prescription

A prescription should identify the doctor and the prescribed medicines. Ask for the doctor’s data to be completed clearly; a doctor’s stamp may be used in place of handwritten details.
